PRICE TRANSPARENCY
Cost Estimates
Cost estimates are non-personalized estimate of costs that may be incurred by the patient for anticipated services.  Patients can request a cost estimate at any time for their planned procedure and be provided that estimate within 7 business days.
Patients can request a personalized statement based on the average payment received for the anticipated service bundle.
Patients are encouraged to contact their health insurer or HMO for anticipated cost sharing responsibilities.
Actual costs may exceed the provided estimate due to variable factors that may occur after admission into the facility.
Patients will receive a  statement from North Florida Surgery Center for facility fees.  This is the only statement the patient will receive from North Florida Surgery Center.  This statement is for services rendered by the facility including, but not limited to, procedural supplies, professional staffing services (registered nurses, surgical technicians and radiology technologists).
Patients will receive a statement from other health care providers who will bill separately for their services.  This includes professional services (from your surgeon's office), anesthesia services, pathology services and radiology services (if applicable).
Patients can contact their surgeon's office directly for more information about expected professional service fees.  Patients can also go to Meet our Physicians page to learn more.
Patients may pay less for services at another facility or in another healthcare setting.  Check with your provider on other facilities they maybe credentialed with for more information.

Financial Assistance
Patients will be provided with their facility co-payment or co-insurance amount prior to their procedure date. 
Co-payments or co-insurance will be due at time of service.
Uninsured and cash pay patients will be required to make payment arrangements from to the procedure date. Patients maybe eligible for a cash pay discount from the facility.
Patients who are experiencing financial difficulty may contact our office to discuss financial assistance options including payment plans by calling (850) 494-0048.
North Florida Surgery Center accepts all major credit cards and Care Credit.  We do not accept personal checks.
Patients can apply for Care Credit, a third party financing company, by visiting their website at www.carecredit.com or calling directly at (800) 365-8295. Subject to credit approval

Billing Practices
Patients will receive a billing statement after discharge from North Florida Surgery Center.
The billing statement will include: contact information for healthcare services that will be billed separately and contact information for billing questions or disputes.
Patients will also receive an EOB or Explanation of Benefits from their health insurance explaining any amount paid for services from the carrier and any amount due from the patient.
Patients are expected to comply with their financial obligations in a timely manner including paying the estimated portion by the day services are rendered. 
The facility will attempt to reach the patient to obtain any outstanding balance amounts. 
Delinquent accounts maybe sent to a third party collections agency or attorney.
